Collaboration is key

The Transition Outreach will work collaboratively with various community supports and resources in order to provide the youth with opportunities to gather the skills and knowledge they will need for independent living, maintain their progress on their predetermined outcomes, and connect each youth to their community.

For youth, by youth

Youth can learn skill building in a variety of areas identified by each youth and may include basic life skills such as learning to cook, grocery shop, budgeting, gathering identifications, on-line courses, job readiness preparation and workshops to explore educational and career paths.

Changing lives through youth-directed programs

The needs of children, youth and their families are the primary focus in the design and delivery of our programs and services.
